The MOSAIC Community Clinic (MCC) operates in the Burnaby Edmonds neighborhood on the traditional lands of the hən̓q̓əmin̓əm̓ and Sḵwx̱wú7mesh Sníchim peoples. MCC serves as a medical home for Burnaby’s diverse priority populations , providing ongoing primary care to:

  • Newcomers (in Canada for less than 3 years)
  • Persons who identify as First Nations, Inuit, and Métis
  • People living with complex mental health issues
  • People experiencing homelessness or precarious housing.

Routine checkups: Health assessments to monitor overall health status, including preventative care, health education and anticipatory guidance.

This Burnaby Primary Care Network clinic is operated by MOSAIC, in partnership with Fraser Health, the Burnaby Division of Family Practice, the Burnaby Inter-Agency Council and the Ministry of Health.

The Burnaby Primary Care Network (PCN), a community-based network of health-care professionals and clinics who plan and deliver the health-care needs of a community. PCNs are part of BC’s Primary Care Strategy that focuses on implementing team-based care.

The MCC team consists of:

  • Registered Nurses

All our practitioners are supported by Medical Office Assistants, a Clinic Manager, Clinic Director, and Medical Director.

Health assessments to monitor overall well-being, Including routine screenings and health education.

Immunizations

In-clinic immunization and mobile seasonal vaccine clinics.

Chronic Disease Management

Ongoing care and support for conditions like diabetes, hypertension, and asthma.

Women’s and Children’s Health

Reproductive health, post-partum, lactation support, and pediatric care.

Referrals to Specialists

Coordination of care with other healthcare providers when needed.

Patient Education

Coordination of care with other healthcare providers when needed.

How To Refer To The MCC

The MOSAIC Community Clinic accepts referrals from the Provincial Health Registry (managed in Burnaby by the Burnaby Division of Family Practice), community partners, and self-referrals.

Patients with valid MSP, the Burnaby Division of Family Practice pre-screens registrants from the Provincial Health Registry and assigns to the MOSAIC Community Clinic. Therefore, individuals are encouraged to ensure they are registered with the ⁠Provincial Health Registry.

Registration takes less than five minutes and requires a Personal Health Number (PHN), home address, email, and phone number. We would be happy to assist anyone who is having difficulty accessing or completing the Health Registry application. Contact us at communityclinic@mosaicbc.org.

Individuals without valid MSP can self-refer using the attached form or have a community organization do so on their behalf. Please complete the form as thoroughly as possible; however, we will not reject incomplete submissions. At a minimum, we require the individual’s name and contact information, and we will follow up to gather any missing details. Referral forms can be emailed, faxed, or dropped off at the clinic. Contact information is at the top of the form.

Referred individuals must identify with one of our priority populations and be seeking ongoing primary care; priority is given to Burnaby residents.

If immediate healthcare needs arise while referrals are being processed, individuals should continue using walk-in clinics or go to the nearest Urgent Primary Care Centre as appropriate.

 Parking 

Enjoy 2-hour free parking! You can access it from the front of the Edmonds Centre for Healthy Communities, next to Price Smart Foods. Patients can park in any unreserved parking stall. Head up to the third level and follow the signs to the Fitness Centre. The clinic’s entrance is just off the parkade to your left.

 Street Access 

For those arriving on foot, you can access the clinic through the front of the building on Edmonds Street or by entering at the doors marked ‘7366 Edmonds’ near Buy Low Foods. Take the elevator to the 3rd floor, turn left, and you’ll find the clinic just around the corner on the right, at unit 306.

 Public Transport

The nearest SkyTrain station is Edmonds Station, a pleasant 17-minute walk away. If you prefer, bus 106 (New West Station) stops right outside Edmonds Healthy Communities in Burnaby. Buses 119 (Metrotown Station) and 133 (Holdom Station) stop at Walker & Kingsway (in front of WorkBC), requiring a short walk down Edmonds to Linden

Our team is planning an upcoming online workshop designed to help you feel more confident and prepared for your breastfeeding journey. This session will cover key information and offer supportive guidance for soon-to-be parents.

FAQ

What can I expect when I get there?

You will be welcomed into the clinic by one of MOSAIC’s medical office assistants.  You will be asked to meet with our intake clinician (nurse or social worker) who will do a thorough assessment of your health needs.  The intake clinician will assign you to Primary Care Provider (physician or nurse practitioner) who will see you for your health needs and any necessary follow-up.

Do you do virtual appointments?

Online and phone appointments will be available for follow-up only. First time patients need to be seen in the clinic.

Where do I go if I can’t get into the MOSAIC Clinic?

If you are not eligible for services at the MOSAIC Clinic, clinic staff may assist in redirecting you to the most appropriate service.

How does the clinic ensure culturally safe care?

At MOSAIC Community Clinic all staff are trained in providing trauma and violence informed care (TVIC).  Cultural safety is a pillar of TVIC. Professional interpretation services are available to all who require it to overcome language barriers and ensure that all patients feel respected and understood. Our staff is trained in cultural safety, and we tailor our services to meet the unique health needs of each patient, considering their cultural background, lifestyle, and health literacy level.

What should I bring to my first appointment at the clinic?

For your first visit to the MOSAIC Community Clinic, kindly bring any form of identification, your health insurance details if you have them, and any medical records that can assist us in understanding your health history. If you’re on any medication, please bring it along or provide a photo of the container showing the dosage and instructions.

How much does it cost to go to the MOSAIC Community Clinic?

The MOSAIC Community Clinic provides services at no cost to our patients. However, there may be costs for medications and testing at labs outside of the MCC.
